Lakeview Family Dental Plan
Lakeview Family Dental Plan (LVFD Plan) is a simple and affordable alternative to traditional insurance policies. Lakeview Family Dental developed this plan to eliminate premiums and third-party involvement. With LVFDP there are no claim forms, approvals, or yearly deductibles.
Our goal is to help make your dental visits more affordable so you can get the quality dental care you deserve.
With a one-time yearly payment of $349, you are eligible to use LVFD Plan for 12 months (365 calendar days) from the effective date. Membership includes routine prophylaxis treatment every 6 months and discounts towards additional treatments. LVFDP is designed to cover preventative services.

A treatment plan will be printed reflecting our standard fee versus your expense after your discount is applied.
No, Lakeview Family Dental covers 100% of all lab fees/costs.
No, re-sale items such as take-home bleach and toothbrushes are not included with the membership plan.
Yes. Due to the nature of dentures and the lab costs associated with the technique sensitive procedures, a 15% reduction on fees will be given.
Plan limitations and exclusions:
*Prophylaxis is limited to every 6 months in a plan year. Additional prophylaxis will be charged per the discounted amount of 20%
Coordination of Lakeview Family Dental Plan discounts with other insurance plans is excluded. Discounts do not apply to other dental insurance coverage, plans, or payments.
Patients are required to have a set of bitewing x-rays once per year.
We cannot be held liable if treatment is unable to be completed due to members general health or physical condition.
Membership of the LVFD Plan benefits cannot be assigned or extended to another person. Membership fee is due at time of enrollment in full. No refunds will be issued for the membership fee if not used in full.
Any treatment referred to a specialist office will not be eligible for any discount on services.
Any dental procedure in progress or preformed before or after a membership term is excluded.
*Prophylaxis preventative treatment designed to preserve dental health and to prevent the spread of disease. Most commonly referred to as the “cleaning” portion of your routine visit.
